We provide all direct support needed to achieve and maintain independence in the community. ExcellentCare provides direct support and assistance for participants, with or without the caregiver present, in or out of the participant’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in the individual’s Service Plan. Community Based Supports are delivered one-on-one with a participant and may include but are not limited to:

• Assistance with community-based activities and assistance to, as well as training and supervision of individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
• Support from our dedicated staff enables an individual to attend events and participate in activities such as:
• Medical or Psychiatric Appointments
• assistance in completing activities of daily living
• ordering off a menu,
• Shopping and purchasing items,
• learning basic cooking
• laundry skills
• etiquette
• travel training
• accessing activities in the community
• Community Activities of the individual’s choices planned that week
• Cultural Events
• Sporting Events

• Accompanying to classes, program, dance, movies and cooking classes
• Activities of Daily Living in the home and in the community
• Trips to amusement parks, bowling, restaurants, etc.
• Travel training
• Money management assist with shopping.
• One-to-One Tutoring Services
• Assist with Doctors appointments and other errands’ etc.
• Supports with leisure activities such as community movies, and dining out

• Skill building activities such as cards games, board games, gaming tournaments
• Civic activities that promote inclusion and contribution to the communities in which individuals reside
• Summer enrichment activities that include daily and weekly camps
